Welcome to the Park Hill Health Office
- Please have your pediatrician fax in updated physical forms and immunizations annually
- Notify the nurse of all medication, food, insect/animal and environmental allergies
- All medication to be given in school will require a parent and physician permission form
- Please have an adult hand medication to the nurse, do not send medication in with your child
- Please have your child wear closed toe shoes for gym/playground
- Send in an extra set of clothing for bathroom accidents, spills and wet playgrounds
- Have a plan for sick days and who will pick up your child if they get sick at school
